Summary of the Oregon State Guidelines

1. Required Screenings

In the state of Oregon, schools conduct routine vision screenings for early detection of visual concerns. Screenings are scheduled across preschool and multiple grade levels to ensure timely intervention.

Screening Grades:

  • Preschool (Ages 3–5)
  • Kindergarten
  • Grades 1, 2, or 3
  • Grades 4 or 5
  • Grades 7 or 8
  • Grades 10 or 11

Preferred Methods:

  • Distance central visual acuity testing using the Snellen chart
  • Lazy E Chart: Recommended for Grades 2 and younger
  • Alphabet Chart: Used from Grade 3 upward unless there are language or developmental concerns

Color Vision Screening:

  • Suggested: Conducted in Kindergarten and prior to high school

2. Screening Referral Criteria (Criteria for Passing Vision Test)

  • Distance Visual Acuity: Must align with age-appropriate referral standards
  • Avoid over-referral: Do not use machines like Titmus or Keystone, which tend to result in unnecessary referrals
  • Near Visual Acuity: Considered less accurate and not preferred

3. Children Who Should Bypass Screening

  • Children with existing vision diagnoses or under ongoing care of an eye care professional
  • Children wearing corrective lenses
  • Children with language or developmental barriers — alternative assessments may be needed

  • Occluders – plastic, flip-up, or frosted
  • Measuring tools – 10-foot cord or 12-foot tape measure
  • Penlights
  • Stickers for children post-screening
