Summary of the Minnesota State Guidelines

1. Required Screenings

  • Visual acuity screeningfor clarity of vision at specified ages:
    • Ages 3–6 years: LEA SYMBOLS® or HOTV charts
    • Ages 6+ years: Sloan letters chart
  • Binocular Fix and Follow for children 4 months to 3 years
  • Corneal Light Reflex to check for constant strabismus
  • Unilateral Cover Test:
    • At Near: Ages 6 months–20 years
    • At Distance: Ages 3 years–20 years
  • Plus Lens Screening for hyperopia (ages 5+)
  • Color Vision Deficiency Screening for kindergarten males
  • Stereo Acuity Screening for children ages 3–8 or up to grade 3
  • External Inspection and Observation for eye health

2. Screening Referral Criteria (Criteria for Passing Vision Test)

Visual Acuity Passing Criteria:

  • Age 3: 10/25 (20/50) or better in each eye
  • Age 4: 10/20 (20/40) or better in each eye
  • Ages 5+: 10/16 (20/32) or better in each eye
  • Referral: If there's a difference of two or more lines between eyes

Plus Lens Screening:

  • Pass: Blurred vision with +2.50 glasses
  • Refer: Clear vision with +2.50 glasses

Other Screening Criteria:

  • Binocular Fix and Follow: Eyes follow the target without abnormal movements
  • Cover Test (Near/Distance): No repeatable movement when the eye is uncovered
  • Corneal Light Reflex: Light reflections are symmetrical
  • Stereo Acuity: Ability to identify depth perception symbols
  • Color Vision Screening: Failing Ishihara plates or similar tests results in referral

3. Children Who Should Bypass Screening

  • Children with known eye conditions under care of an ophthalmologist/optometrist
  • Children already wearing glasses, unless a current prescription check is needed
  • Untestable children after two rescreen attempts should be referred for a full eye exam

4. Required Tools OR Important Changes to Tools Used/Discontinued

Tools Required:

  • LEA SYMBOLS®, HOTV charts, and Sloan Letters for visual acuity
  • Plus Lens (+2.50 glasses) for hyperopia screening
  • Occluder glasses, plastic occluders, and adhesive patches to prevent peeking
  • Color Vision Screening: Ishihara plates or “Color Vision Testing Made Easy”
  • Stereoacuity cards (e.g., Random Dot E or Butterfly Test)
