A pediatric eye chart featuring HOTV optotypes for screening/testing the vision of children and adults 3 years and older. This eye chart folds in half for added mobility or easy storage. The front side of the eye chart features HOTV characters in proportionally-spaced lines (ETDRS, format) for monocular and binocular (left eye / right eye) vision testing. On the opposite side, the HOTV characters are in Massachusetts Visual Acuity Format for both left eye and right eye testing.
The proportionally-spaced line side features line sizes ranging from 10/100 to 10/5 (3/30 to 3/1.5) equivalent. The 50% spaced rectangle (Massachusetts Visual Acuity Test format) side features line sizes from 10/50 to 10/4 (3/15 to 3/1.2) equivalent. Has a hole for hanging on the wall. Includes a response key and instructions.
