Transparent Target Forms

Bullseye targets based on procedures developed by Lawrence W. MacDonald, OD and Baxter Swartwout, OD. Each comes in a pack of 5 translucent forms.

Form A is a set of 5 clear bullseye targets measuring 2" x 3 1/2"

Form B is a set of 5 clear targets with five lines of words appearing above a bullseye, each line decreasing in font size, and two lines of words below the bullseye. Each target measures 3" x 3 1/2"

Form C is a set of 5 clear targets with three lines of letters below the bullseye. Each target measures 1 1/2" x 3 1/2".
