1. Required Screenings

In the state of West Virginia, vision screenings are a critical part of student health monitoring, with priority given to key age groups and high-risk students.

  • All first-time school enterers
  • Children aged 11–13 years
  • Priority screenings: For students referred by teachers, parents, or self-referred
  • High-risk groups:
    • Special education students
    • Students repeating a grade
    • Students who previously failed a screening without follow-up
  • Additional screenings: Color vision or near vision if concerns are raised
  • Optional: Screening additional grades if resources and time allow

2. Screening Referral Criteria (Criteria for Passing Vision Test)

  • Age 3: Refer if 20/50 or worse in either eye
  • Age 4: Refer if 20/40 or worse in either eye
  • Age 5 and older: Refer if 20/30 or worse in either eye
  • Any age: Refer if there is a two-line or greater difference between eyes

3. Children Who Should Bypass Screening

  • Students with known vision impairments that are already diagnosed and managed
  • Students currently under eye care from an optometrist or ophthalmologist

Recommended Distance Vision Tools

  • LEA SYMBOLS® Chart: For pre-language children (ages 3–4) and kindergartners
  • Snellen E Chart: For school-aged children
  • Adolescent screening: Other validated evidence-based tools

Color Vision Tools

  • Ishihara Color Vision Test Book: 10-plate or 14-plate versions
  • Color Check Complete Vision Screener

Occluders
