Summary of the Arizona State Guidelines

1. Required Screenings

  • Annual Screenings:
    • Children ages 9 and younger: Preschool, Kindergarten, 1st, 2nd, 3rd, and 4th grades.

  • Biannual Screenings:
    • Children ages 10 and older: 5th grade and up.

2. Screening Referral Criteria (Criteria for Passing Vision Test)

  • Distance Visual Acuity Tests:
    • HOTV Letterso LEA Symbols
    • Sloan Letters (used once a child is 6 years or older).

  • Near Vision Acuity Tests:
    • HOTV Letters
    • LEA Symbols.

  • Stereoacuity Tests:
    • Random Dot "E"
    • Butterfly Test.

  • Color Deficiency Screening:
    • Conducted only once using Pseudoisochromatic Plates.

  • Instrument-Based Screening (Photoscreeners/Autorefractors):
    • For ages 1–6 years or children over 6 years who cannot participate in chart-based screenings.

    • Approved Tools:
      • Welch Allyn® Spot™ Vision Screener
      • Plusoptix S12C Vision Screener
        • Retinomax (Right Mfg. Co Ltd.)
        • SureSight Vision Screener (Welch-Allyn, Software v2.25).

3. Children Who Should Bypass Screening

  • Children with:
    • Documented vision impairment diagnoses.
    • Previously identified significant visual issues.
    • Ongoing eye care under a professional (e.g., ophthalmologist, optometrist).

4. Required Tools OR Important Changes to Tools Used/Discontinued

  • Required Tools:
    • Distance Visual Acuity Charts: HOTV, LEA Symbols, Sloan Letters.
    • Near Vision Acuity: HOTV and LEA Symbols.
    • Stereoacuity Tools: Random Dot "E" and Butterfly Test.
    • Color Deficiency Plates: Pseudoisochromatic Plates.
    • Instrument-based Tools: Welch Allyn® Spot™, Plusoptix S12C, Retinomax, SureSight Vision Screener.

  • Important Notes:
    • Instrument-based Screening is encouraged for younger children (ages 1–6) or those unable to participate in traditional screenings.
    • There is no specific mandatory rule in place for vision screening training, tools, or reporting for the 2022-2023 school year as per state regulations.
