Summary of the Hawaii State Guidelines

1. Required Screenings

Visual Acuity: 

  • Distance visual acuity using age-appropriate optotypes (e.g., LEA SYMBOLS® or HOTV for ages 3–5, Sloan Letters for ages 6+).
  • Monocular screening (one eye at a time) at standardized distances (5 or 10 feet depending on the chart).

Depth Perception (Stereopsis):

  • Screening for stereopsis (depth perception) is part of GLD-Vision, but not always included in traditional screening.

Color Vision Deficiency:

  • GLD-Vision includes color vision screening; traditional screenings often exclude this test.

Instrument-Based Screening (if applicable):

  • For ages 1–2 years or children who cannot participate in optotype-based screening.

Periodicity:

  • Annual screenings are recommended for ages 3–6 years and at specific intervals thereafter (ages 8, 10, 12, and 15) based on AAP guidelines.

2. Screening Referral Criteria (Criteria for Passing Vision Test)

  • Ages 3–5: Majority of optotypes must be identified on the 20/50 line for 3-year-olds and 20/40 line for ages 4–5.
  • Ages 6+: Majority of optotypes on the 20/32 line.
  • Near Vision: Pass criteria similar to distance vision (varies by age and chart used).
  • Instrument-Based Screening: Results indicating refractive errors or eye misalignment would trigger a referral.

3. Children Who Should Bypass Screening

Children with Existing Diagnoses or Vision Correction:

  • Those who wear glasses or contact lenses (should be referred directly for a comprehensive eye exam).
  • Children under active care of an eye care professional.

Young Children Unable to Participate:

  • For children who cannot perform optotype-based screening, instrument-based screening is recommended.

4. Required Tools OR Important Changes to Tools Used/Discontinued

Required Tools:

  • Charts:
    • LEA SYMBOLS® or HOTV (ages 3–5) for distance visual acuity.
    • Sloan Letters or LEA Numbers® (ages 6+).

  • Occluders:
    • Acceptable: Frosted lens occluders, flip-up glasses, and spectacle occluders.
    • Unacceptable: Hands, tissues, paper cups, or adults holding occluders.

  • Instrument-Based Screeners:
    • Devices like Welch Allyn Spot or Plusoptix for automated screening.

  • Near Vision Charts:
    • Charts with measuring cords for proper positioning.

  • Depth Perception and Color Vision Tools:
    • GLD-Vision integrates these tests automatically.

  • Important Changes:
    • GLD-Vision:
      • Eliminates the need for traditional charts, occluders, and manual data entry.

    • Traditional Screening:
