Summary of the Nevada State Guidelines

1. Required Screenings

In the state of Nevada, vision screenings are performed to identify early signs of vision problems in preschool and school-aged children. These screenings support educational success and developmental health.

  • Visual Acuity Screening:
    • Ages 3–5: LEA Symbols® or HOTV Letters at 5 feet.
    • Ages 6+: Sloan Letters or LEA Numbers® at 10 feet.
    • Conducted monocularly (one eye at a time).
    • Annual screening recommended for preschool-age children.
  • Near Vision Screening: Sloan Letters or LEA Numbers® near charts at 16 inches.
  • Instrument-Based Screening: Recommended for ages 1–2 or children unable to complete standard screenings.
  • Optional Screenings:
    • Stereoacuity (depth perception)
    • Color vision (e.g., Ishihara test)

2. Screening Referral Criteria (Criteria for Passing Vision Test)

  • Visual Acuity (Distance):
    • Age 3: Majority on 20/50 line
    • Ages 4–5: Majority on 20/40 line
    • Ages 6+: Majority on 20/32 line
  • Near Vision Screening:
    • Age 3: 20/50
    • Age 4–5: 20/40
    • Age 6+: 20/32
  • Instrument-Based Screening: Refer if signs of refractive error or misalignment appear.
  • Color Vision: Refer for color deficiency if failed.
  • Stereoacuity: Pass/fail per test protocols (e.g., PASS Test).

3. Children Who Should Bypass Screening

  • Children with documented vision impairments or under current vision care.
  • Children already wearing prescription glasses or contacts.
  • Children who have had a recent eye exam by an eye care provider.
  • Those with behavioral or physical limitations that prevent valid testing.
  • Older children who are able to use optotype-based methods should not use instrument-based screening.

4. Required Tools OR Important Changes to Tools Used/Discontinued

Accepted Tools:

  • LEA Symbols®, HOTV Letters (ages 3–5)
  • Sloan Letters, LEA Numbers® (ages 6+)
  • Near vision charts with 16-inch measuring cords

Acceptable Occluders:

  • Frosted Lens Occluders
  • Eye Patches
  • Flip-Up Frosted Glasses (ages 3–10)
  • Mardi Gras or “Lollypop” occluders (ages 10+)

Instrument-Based Screening Devices:

  • Welch Allyn Spot Vision Screener
  • Plusoptix S12C

Color Vision and Depth Tools:

  • Ishihara 10- or 14-plate books
  • PASS Test for stereoacuity

Important Notes:

  • Unacceptable Occluders: Tissues, hands, paper/plastic cups
  • GLD-Vision®: Encouraged for digital efficiency, validated accuracy, and reduced admin load
