Summary of the Maryland State Guidelines

1. Required Screenings

Timing of Screenings:

  • When a student enters a school system.
  • In the year a student enters first grade.
  • In the year a student enters eighth or ninth grade.

Vision Screening Types:

  • Monocular Distance Visual Acuity:
    • Tools: LEA SYMBOLS®, HOTV Letters, Sloan Letters
    • Ages:
      • LEA SYMBOLS® or HOTV: Recommended for ages 3–6 years
      • Sloan Letters: Recommended for ages 6 years and older
  • Instrument-Based Screening(Refractive error/eye misalignment):
    • Tools: Righton Retinomax, SureSight Vision Screener, Welch Allyn Spot Vision Screener, Plusoptix
    • Ages: Recommended for ages 3–5 years

Optional Screenings:

  • Stereoacuity:
    • Tools: PASS Test 2, Random Dot “E”
  • Color Vision:
    • Tools: Ishihara plates, Color Vision Testing Made Easy, or similar

2. Screening Referral Criteria (Criteria for Passing Vision Test)

Monocular Distance Visual Acuity:

  • 3 years: Unable to pass 20/50 line in either eye
  • 4 years: Unable to pass 20/40 line in either eye
  • 5+ years: Unable to pass 20/32 line in either eye
  • Any age: A two-line difference between eyes (e.g., 20/16 and 20/25)

Instrument-Based Screening:

  • Follow specific manufacturer guidelines for each tool

Stereoacuity Tests:

  • PASS Test 2 (3–4 years): Unable to identify the Smile face on Card B
  • PASS Test 2 (5+ years): Unable to identify the Smile face on Card B or C
  • Random Dot “E”: Unable to see the E figure at all or only at close distance (~20 inches)

Color Vision Tests:

  • Failure: Unable to identify the required number of plates determined by the tool’s manufacturer

3. Children Who Should Bypass Screening

  • Students who already have a diagnosed vision impairment or wear corrective lenses (glasses/contacts)

4. Required Tools OR Important Changes to Tools Used/Discontinued

Required Tools:

  • Monocular Distance Visual Acuity: LEA SYMBOLS®, HOTV, Sloan Letters
  • Instrument-Based Tools: Welch Allyn Spot Vision Screener, Plusoptix, Righton Retinomax, SureSight Vision Screener
  • Stereoacuity Tools: PASS Test 2, Random Dot “E”
  • Color Vision Tools: Ishihara plates, Color Vision Testing Made Easy

Important Notes:

  • Emphasis on instrument-based screening for younger children (ages 3–5) who may have difficulty with optotype-based charts
  • Referral criteria ensure consistent identification of vision issues in Maryland State
