Summary of the Rhode Island State Guidelines

1. Required Screenings

In the state of Rhode Island, vision screenings are mandated upon school entry and during specific grade levels: preschool, kindergarten, 1st, 2nd, 3rd, 4th, 5th, 7th, and 9th.

  • Screening methods: Must be age-appropriate optotype-based or instrument-based tools
  • Standards followed: National Center for Children’s Vision and Eye Health guidelines
  • Timeframe: Screenings should be conducted early in the school year, and for preschool/kindergarten, must be completed within 30 days of school start

2. Screening Referral Criteria (Criteria for Passing Vision Test)

  • Age 3: Refer if child fails to correctly identify 3 out of 5 symbols on the 20/50 line in each eye
  • Ages 4–5: Refer if child fails to identify 3 out of 5 symbols on the 20/40 line in each eye
  • Age 6+: Refer if child fails to identify 3 out of 5 symbols on the 20/32 line in each eye
  • Any age: Refer if there is a two-line difference between the eyes (e.g., 20/20 vs. 20/32)

3. Children Who Should Bypass Screening

Children with a history of significant vision issues or who are at higher risk of visual problems may bypass regular screenings. In such cases, they should be referred directly to an eye care provider, as determined by the school nurse.

4. Required Tools OR Important Changes to Tools Used/Discontinued

Recommended Tools:

  • LEA SYMBOLS® and HOTV charts: For preschool, kindergarten, and 1st grade students who cannot recognize letters
  • Sloan Letters chart: For students in grades 2–5, 7, and 9
  • Computer-based screenings: Acceptable if they use Sloan Letters

Instrument-Based Screening:

  • Alternative for ages 3–5 or children who cannot complete traditional screening methods
