1. Required Screenings
In the state of Nevada, vision screenings are performed to identify early signs of vision problems in preschool and school-aged children. These screenings support educational success and developmental health.

Visual Acuity Screening:
- Ages 3โ5: LEA Symbolsยฎ or HOTV Letters at 5 feet.
- Ages 6+: Sloan Letters or LEA Numbersยฎ at 10 feet.
- Conducted monocularly (one eye at a time).
- Annual screening recommended for preschool-age children.
- Near Vision Screening: Sloan Letters or LEA Numbersยฎ near charts at 16 inches.
- Instrument-Based Screening: Recommended for ages 1โ2 or children unable to complete standard screenings.

- Stereoacuity (depth perception)
- Color vision (e.g., Ishihara test)
2. Screening Referral Criteria (Criteria for Passing Vision Test)

Visual Acuity (Distance):
- Age 3: Majority on 20/50 line
- Ages 4โ5: Majority on 20/40 line
- Ages 6+: Majority on 20/32 line

Near Vision Screening:
- Age 3: 20/50
- Age 4โ5: 20/40
- Age 6+: 20/32
- Instrument-Based Screening: Refer if signs of refractive error or misalignment appear.
- Color Vision: Refer for color deficiency if failed.
- Stereoacuity: Pass/fail per test protocols (e.g., PASS Test).
3. Children Who Should Bypass Screening
- Children with documented vision impairments or under current vision care.
- Children already wearing prescription glasses or contacts.
- Children who have had a recent eye exam by an eye care provider.
- Those with behavioral or physical limitations that prevent valid testing.
- Older children who are able to use optotype-based methods should not use instrument-based screening.
4. Required Tools OR Important Changes to Tools Used/Discontinued
Accepted Tools:
- LEA Symbolsยฎ, HOTV Letters (ages 3โ5)
- Sloan Letters, LEA Numbersยฎ (ages 6+)
- Near vision charts with 16-inch measuring cords
Acceptable Occluders:
- Frosted Lens Occluders
- Eye Patches
- Flip-Up Frosted Glasses (ages 3โ10)
- Mardi Gras or โLollypopโ occluders (ages 10+)
Instrument-Based Screening Devices:
- Welch Allyn Spot Vision Screener
- Plusoptix S12C
Color Vision and Depth Tools:
- Ishihara 10- or 14-plate books
- PASS Test for stereoacuity
Important Notes:
- Unacceptable Occluders: Tissues, hands, paper/plastic cups
- GLD-Visionยฎ: Encouraged for digital efficiency, validated accuracy, and reduced admin load
