The clinical urgency has grown alongside screen time. Children using digital devices for more than three hours daily show significant reductions in accommodative facility and positive fusional vergences compared to low-use peers, according to a 2022 study published in PLOS One. Accommodative facility (AF) testing is no longer reserved for the symptomatic workup; it belongs in every pediatric encounter.
Consider the child who reads comfortably in class but reports headaches by mid-afternoon. A 60-second AF test can explain the disconnect. Accommodative amplitude (how much the eye can accommodate) and accommodative facility (how quickly and accurately focus shifts occur) are distinct measures, and both are essential for a complete clinical picture. Always test monocularly first (MAF), then binocularly (BAF). Interpreting MAF vs. BAF: What the Discrepancy Tells You
Scheiman and Wick established the standard diagnostic thresholds: MAF below 7 cpm and BAF below 5 cpm indicate inadequate accommodative facility in children aged 8โ12. These cut-offs, grounded in Scheiman et al.'s landmark 1988 normative study of 542 schoolchildren, remain the most commonly cited benchmarks.
The MAF-to-BAF discrepancy carries specific diagnostic meaning. If MAF is normal but BAF is reduced, vergence dysfunction is the primary driver. If both are reduced, accommodative dysfunction is central and should be addressed first.

Refractive status also matters. Myopic children aged 6โ8 show significantly reduced distance AF (5.5 ยฑ 2 cpm) compared to emmetropes (6.9 ยฑ 1.7 cpm), though near AF does not differ significantly between the two groups. Factor refractive error into your interpretation.
Accommodative Amplitude: Hofstetter's Formula and Its Modern Limitations
Hofstetter's formulas have been the clinical standard since 1950. Average amplitude of accommodation (AoA) is calculated as 18.5 โ (0.30 ร age), and the minimum as 15 โ (0.25 ร age). Accommodative insufficiency is traditionally diagnosed when measured AoA falls โฅ2.00 D below the minimum, as documented in standard clinical references.
However, a 2023 meta-analysis found that Hofstetter's formula significantly overestimates AoA in modern school-aged children. Six-year-olds measured 3.4 D below prediction (p=0.025), 9-year-olds 4.1 D below (p=0.043), 10-year-olds 4.6 D below (p=0.035), and 11-year-olds 5.2 D below (p=0.005). The practical implication is significant: clinicians applying Hofstetter's minimum as a strict diagnostic threshold may be over-diagnosing AI in younger children whose true normative amplitude is lower than 1950 predictions.
Measurement method also critically affects results. A 2025 normative study of 409 young adults (mean age 21.44 years) found mean monocular AoA of 15.74 D via push-up but only 10.13 D via minus-lens-to-blur.
