Summary of the Wyoming State Guidelines
1. Required Screenings
In the state of Wyoming, comprehensive vision screenings are conducted at multiple grade levels using validated tools and techniques.

Kindergarten (or first entry into district):
- Monocular Distance Visual Acuity (HOTV optotype wall chart at 10 feet)
- Binocular Near Visual Acuity (HOTV near chart with cord)
- Stereoacuity (PASS Test 2)
- Color Vision (Color Vision Testing Made Easy)

Grades 1, 2, 3, 5, 7, and 10:
- Monocular Distance Visual Acuity (SLOAN optotype wall chart at 10 feet)
- Binocular Near Visual Acuity (SLOAN near chart with cord)
- Stereoacuity (PASS Test 2)
Instrument-Based Screening: May be used for children 6 years and older who cannot complete optotype-based screening.
2. Screening Referral Criteria (Criteria for Passing Vision Test)
Monocular Distance Visual Acuity
- Kindergarten: Refer if unable to identify 3 of 5 optotypes on 20/40 line or if there’s a 2-line difference between eyes
- Grades 1–12: Refer if unable to identify 3 of 5 optotypes on 20/32 line or if there’s a 2-line difference between eyes
Binocular Near Visual Acuity
- Kindergarten: Refer if unable to identify 3 of 5 optotypes on 20/40 line
- Grades 1–12: Refer if unable to identify 3 of 5 optotypes on 20/32 line
Stereoacuity (PASS Test 2)
- Refer if unable to pass all test cards (B and C)
Color Vision
- Refer for any incorrect responses based on manufacturer instructions
3. Children Who Should Bypass Screening
- Recognized eye abnormalities (e.g., strabismus, ptosis)
- Diagnosed neurodevelopmental disorders (e.g., cerebral palsy, autism)
- Systemic diseases affecting vision (e.g., diabetes, juvenile rheumatoid arthritis)
- Family history of strabismus, amblyopia, or high refractive error in a first-degree relative
- Premature birth or low birthweight without a prior normal eye exam
- Parental concerns about vision or developmental milestones
- Students with an IEP or 504 Plan: Screening may still be attempted, but referral for comprehensive exam is encouraged
4. Required Tools OR Important Changes to Tools Used/Discontinued
Required Tools
- Distance Charts: HOTV (Kindergarten), SLOAN (Grades 1–12)
- Near Charts: HOTV or SLOAN charts with cord
- Stereoacuity Test: PASS Test 2
- Color Vision: Color Vision Testing Made Easy or School Health HRR Color Screening Book
- Acceptable Occluders: Handheld paddles, adhesive patches, or occluder glasses
- Unacceptable Occluders: Paper, tissues, hands, or cups
Screening Notes
- Instrument-Based Screening: Not validated for general use with children 6+ unless they are unable to complete optotype testing
- Proper occlusion methods must be used to ensure test validity
