Summary of the South Dakota State Guidelines
1. Required Screenings
In the state of South Dakota, vision screenings are conducted using both traditional optotype-based and modern instrument-based methods to accommodate children across various age groups.
Distance Visual Acuity:
- Ages 3–5: Use LEA SYMBOLS® or HOTV letters
- Tools: Flip charts used at 5 or 10 feet
- Ages 6 and up: Use Sloan Letters or LEA Numbers® with 10-foot standardized charts
Near Vision Screening:
- Ages 3–5: LEA SYMBOLS® or HOTV charts
- Ages 6+: Sloan Letters or LEA Numbers® with 16-inch measuring cord
Instrument-Based Screening:
- Recommended for: Ages 1–2 or children who cannot complete optotype-based tests
Additional Screenings:
- Stereoacuity Screening: Tools like Pass Test 2
- Color Vision Screening: Ishihara plates or Pass Test
2. Screening Referral Criteria (Criteria for Passing Vision Test)
Distance Visual Acuity:
- Ages 3: Must correctly identify majority of optotypes on the 20/50 line
- Ages 4–5: Must correctly identify majority on the 20/40 line
- Ages 6+: Must correctly identify majority on the 20/32 line
Near Vision Screening:
- Same referral criteria as for distance vision by age group
Instrument-Based Screening:
- Referrals based on automated detection of refractive error or ocular misalignment
3. Children Who Should Bypass Screening
- Children with known visual impairments and already under professional eye care
- Children wearing corrective eyewear (e.g., glasses or contacts)
- Children with physical or developmental limitations that prevent testing
4. Required Tools OR Important Changes to Tools Used/Discontinued
Tools for Distance Visual Acuity:
- LEA SYMBOLS®
- HOTV letters
- Sloan Letters
- LEA Numbers®
- Testing distance: 5 feet or 10 feet standardized charts
Near Vision Tools:
- Use of near charts with a 16-inch measuring cord
Occluders:
- Acceptable: Frosted occluders, flip-up glasses, or eye patches
- Unacceptable: Hands, tissues, plastic or paper cups, or improperly held occluders
Instrument-Based Tools:
- Welch Allyn Spot
- Plusoptix S12C
Discontinued Practices:
- Use of tissues or hands as occluders
- Non-standardized or age-inappropriate charts
