Summary of the Ohio State Guidelines
1. Required Screenings
In the state of Ohio, vision screening is required at specific grade levels and under particular conditions to identify children who may need comprehensive eye care.
- Preschool children: Must be screened annually if enrolled in a school-based program.
- School-aged children: Must be screened in Kindergarten, 1st, 3rd, 5th, 7th, 9th, and 11th grades.

Additional required screenings:
- Newly enrolled children without documented prior screenings
- Children absent during the last screening period
- Children referred by school personnel or parents
- Children whose parents request a vision screening
2. Screening Referral Criteria (Criteria for Passing Vision Test)

Visual Acuity:
- Refer if vision is 20/40 or worse in either eye
- Refer if a two-line difference exists between eyes (e.g., 20/32 vs. 20/50) for potential amblyopia
- SureSight Vision Screener: Refer when an asterisk (*) appears on screen
- Stereopsis Tests: Failure to identify PASS 2 or Random Dot E test symbols requires referral
- Color Vision: Referral required if child fails manufacturer’s criteria; parents must be notified
- Ocular health concerns: Issues like pupil abnormalities, tropias, or discharge prompt immediate referral
3. Children Who Should Bypass Screening
- Untestable children: Those who cannot complete screening due to cognitive, physical, or behavioral conditions should be referred for an eye exam
- Children with diagnosed disorders: Includes ADHD, Autism, Down syndrome, systemic diseases, or family history of vision issues
- Existing exam: If a child has had a professional eye exam in the last 12 months
- Parental refusal: Must be documented in writing in the child’s health record
4. Required Tools OR Important Changes to Tools Used/Discontinued
Approved Tools:
- LEA Symbols®: Used at a 10-foot testing distance
- SureSight Vision Screener: Must follow Vision in Preschoolers (VIP) Study criteria
- Retinomax Autorefractor: Especially used in Kindergarten and 1st grade
