Summary of the Idaho State Guidelines

1. Required Screenings

Distance Visual Acuity (Optotype-Based)

  • Ages 3–5: Use single, surrounded optotype flip charts at 5 feet or flip charts with crowded lines at 10 feet.
  • Best practice charts include LEA SYMBOLS® and HOTV letters.
  • Pass Criteria:
    • 3 years: 20/50 line
    • 4–5 years: 20/40 line
  • Ages 6+: Use Sloan Letters or LEA Numbers® charts at 10 feet.
  • Pass Criteria: Majority of optotypes on 20/32 line with each eye.

Near Vision Screening

  • Ages 3–5: LEA SYMBOLS® or HOTV letters.
  • Ages 6+: Sloan Letters or LEA Numbers® with 16-inch measuring cord.

Instrument-Based Screening

  • Suitable for children aged 1–2 years and for those unable to participate in optotype-based screening.
  • Devices include Welch Allyn Spot and Plusoptix S12C.

2. Screening Referral Criteria (Criteria for Passing Vision Test)

Distance Visual Acuity:

  • 3 years old: Must identify the majority of optotypes on the 20/50 line.
  • 4–5 years old: Must identify the majority of optotypes on the 20/40 line.
  • 6+ years old: Must identify the majority of optotypes on the 20/32 line.

Near Vision:

Similar pass criteria as above depending on age.

3. Children Who Should Bypass Screening

  • Children with documented vision conditions or current vision care management.
  • Children with glasses or other corrective lenses should bypass screening, as their vision is already being monitored by an eye care professional.

4. Required Tools OR Important Changes to Tools Used/Discontinued

Recommended Tools:

  • LEA SYMBOLS® and HOTV Charts for ages 3–5.
  • Sloan Letters or LEA Numbers® Charts for ages 6 and up.
  • Instrument-Based Devices for non-cooperative children or younger age groups (Welch Allyn Spot and Plusoptix S12C).
  • Occluders: Frosted lens occluders, eye patches, or “Lollypop” occluders.

Disallowed Tools:

  • Tissues, hands, cups, and paper occluders are unacceptable.

Not Recommended:

  • Near vision, stereoacuity, and color vision screening unless specified.
